We are running a survey to gather people’s views on how well we support people to live their lives their way. This survey is for family members, friends or advocates, or people who provide professional input into the lives of people we support we would love to hear your views. We have a separate one that asks people we support what they think.
The survey is simple and straightforward to complete, and should only take you a few minutes. If you would like to take part, please contact us at feedback@affinitytrust.org and we will send you a link to our online form. You do not need to share personal details when you submit the form if you would prefer not to.
